Start by applying to the National Health Service Corp (NHSC)
The National Health Service Corps (NHSC) is part of the United States Department of Health and Human Services, division of Health Resources and Services Administration, Bureau of Health Workforce. Members are health professionals providing primary health care services in underserved communities since 1972.
Because the NHSC has approved Heartland as a qualified site, we are able to offer our employees loan forgiveness and our clients financial assistance. No one will be denied access to services solely due to inability to pay full price. We will work with anyone willing to fill out an application for assistance. We have a discounted/sliding fee schedule available based on family size and income. See below for sliding scale and other information on client financial assistance. After applying to NHSC, you may submit an application to the Heartland Patient Assistance Fund (PAF) to assist with remaining cost. Print out the form below, fill it out, and submit it to our front desk.
